Children and Young People Long Term Condition Survey
Overview

If you are a young person living with asthma, epilepsy or diabetes, or you are a parent/carer; we want to hear from you.
This survey is part of a wider piece of work looking at the experiences of children and young people with long-term health conditions across North Northamptonshire.
The survey can be completed by young people aged 11 years and over who have not yet reached their 19th birthday and who are living with asthma, epilepsy, or diabetes. It can also be completed by parents/carers whose child or young person has asthma, epilepsy or diabetes. Your feedback will help us understand what is working well, what challenges you may face, and where support could be improved.
By taking part, you will help us build a clearer picture of the experiences of children and young people with long-term health conditions and identify any gaps in support that may affect you achieving the outcomes that matter to you.
Privacy and confidentiality
Your responses will be kept confidential and anonymous. The information you provide will be used to help improve services and support for children and young people. Your response will be anonymous, and the results of the survey may be published in reports by North Northamptonshire Council to help show what people have told us about their experiences. No individual child, young person, parent, carer, or school will be identified, and we will not ask you for any identifiable information, such as your name, date of birth or school.
We may compare responses from different areas of North Northamptonshire (Corby, East Northants, Kettering, and Wellingborough) to help us better understand local needs. Any results shared will be based on combined responses and will not identify any individuals or schools.
Using your comments
With your permission, anonymous comments may be used in reports, presentations, publications, and other communications to help share what we have learned from the survey. Any comments used will not identify you or your child. At the end of the survey, you will be asked whether you agree to your comments being used in this way.
If you are under 16, you may want to complete this survey with a parent, carer or trusted adult, especially if you are unsure about any of the questions

At the end of the survey there is an opportunity, for those responding as a parent/ carer, to share some demographic information. The purpose of asking for equality monitoring information is to check that our consultation and engagement activities have reached an audience that reflects our community and ensure everybody has an opportunity to be heard. It is voluntary and we thank you, in anticipation, for taking the time to share your details.
